Duties & Responsibilities
Assess the psychosocial needs of residents and develop individualized care plans.
Provide counseling and support to residents and their families on issues such as adjustment to facility life, grief and loss, and coping with illness.
Advocate for residents' rights and assist them in accessing community resources and benefits.
Coordinate with healthcare providers, social workers, and other professionals to ensure residents receive comprehensive care.
Develop and implement social and recreational programs to promote resident engagement and well-being.
Educate residents and families on available services and resources. Maintain accurate records and documentation. Stay informed about changes in social service regulations and best practices.
